How to Request Reimbursement from Your Insurance Provider
A Guide from Upwell Therapy
All services are private pay at Upwell Therapy. You may be eligible to get reimbursed through your insurance plan’s out-of-network benefits. While we don’t bill insurance directly, we’re happy to provide you with a superbill—a special receipt with all the information your insurance company needs.
What Is a Superbill?
A superbill is a detailed invoice that shows:
The type of therapy your child received
Diagnosis and procedure codes
How much you paid
Our provider and business details
You can submit this document to your insurance company to request reimbursement.
Steps to Submit Your Superbill
Contact your insurance company
Ask:
“Do I have out-of-network benefits for speech or feeding therapy?”
“What percentage is reimbursed?”
“What’s the process for submitting a superbill?”Make note of your deductible, coverage percentage, and submission method.
Collect your documents
The superbill (we’ll give this to you after each session or monthly)
A copy of your payment receipt (if required)
Submit to your insurance
Most plans allow you to upload through their online portal or mail a form with the superbill.
Use the method your plan prefers (ask them if unsure).
Track your claim
Keep a copy of what you submitted.
Follow up with your insurance if you don’t receive a response within 2–4 weeks.
Tips
Reimbursement is not guaranteed, but many families successfully receive partial or full reimbursement.
Be sure to submit within your insurance plan’s timeline (often 90–180 days).
